Arranging the Reablement Service
This procedure should be used to arrange for a reablement service to start when:
- A proportionate assessment of needs has been carried out; and
- The person has needs that can be prevented, reduced or delayed; and
- Reablement is an appropriate and proportionate way to prevent, reduce or delay those needs; and
- The person wishes to use reablement; and
- A Reablement Plan and clear outcomes for reablement have been agreed.
It is used by all reablement teams /services in Southwark.
When the Reablement Plan has been formally recorded a decision is required about who should provide the reablement service prior to making any arrangements.
This decision should be made with regard for:
- The views of the person about who should provide reablement;
- The views of any carer about who should provide reablement; and
- The impact of the decision on the person's Wellbeing.
Using the information in the Reablement Plan (and with regard for the views of the person, any carer and Wellbeing) you must decide:
- The frequency of reablement visits required at the start of the plan;
- The duration of reablement visits at the start of the plan;
- The timings of the reablement visits at the start of the plan;
- Whether any additional information is required prior to starting reablement (for example information about risk to the worker);
- Whether any requests to work jointly with occupational therapy, assistive technology or social work have been made in a timely way;
- When the reablement service is likely to begin.
If you require any additional information before proceeding to arrange the reablement service you should gather it, seeking the consent of the person where required to do so.
You should contact the person if there are likely to be any delays in starting the reablement service. You should explain:
- The cause of the delay; and
- The anticipated delay time.
When the proposed reablement service is confirmed you must contact the person (or their representative if they lack capacity) to explain:
- The frequency, duration and timing of initial visits;
- That the frequency, duration and timing of initial visits are subject to change as reablement progresses;
- Who will be providing reablement;
- When the reablement service is going to begin; and
- Answer any questions they have about the service, or provide any further information or advice that you feel may be beneficial.
You should also take the opportunity to:
- Reiterate that reablement is a time limited service and is regularly monitored and reviewed; and
- That reablement is free for up to 6 weeks and, if it does continue past this point normal charging rules apply.
If the person advises that the proposed start date for reablement is not convenient for them (for example if they have a medical appointment or family matter) you should discuss and agree a revised date.
You should provide a record of what has been agreed in writing. This can be posted to the person or hand delivered if reablement is scheduled to start imminently.
When reablement is being provider externally you must provide the following information to the agency so that they can plan how best to provide the reablement service:
- A copy of the Reablement Plan;
- With the person's consent, a copy of the assessment;
- Information about the specific skills required of any workers;
- Any information relating to risk; and
- Information and contact details of anyone else involved in the plan (for example occupational therapy, assistive technology or social work).
You should agree with the person and any external provider how the reablement service will be monitored. This should be in line with the arrangements for monitoring and review recorded in the Reablement Plan.
It remains the responsibility of the Local Authority to monitor and review the reablement service.
